Is my service eligible to join the Forum?
The forum executive committee would welcome application for membership from any service providing end of life care at home or planning to set up a H@H service. The membership has highlighted 7 core elements of a hospice at home service as a guide for service model content.
My service is not linked to a hospice, am I still eligible ?
The forum has a wide ranging membership including NHS services and those working in collaboration, usually developed in response to local need.
What are the benefits of joining?
Hospice at Home has become a core service in enabling end of life care at home and the forum has an executive committee which is dedicated to promoting the cause of Hospice at Home at a local, regional and national level.
